What is a Home Care Package?
Home Care Packages provide government funding to help individuals remain living independently at home. These packages offer a coordinated care plan, giving you choice and flexibility over the services you need and want.
Eligibility for Home Care Packages
Home Care Packages are available for:
Anyone needing coordinated care to remain
at home.
Younger people with disabilities, dementia, or special needs not met by other services
To obtain a Home Care Package, you will be assessed by the Aged Care Assessment Team (ACAT), who will determine the appropriate level of support.
Home Care Package Levels
Level 1
Basic Care Need
Approx. Fortnightly Funding
$395.04
This may include assistance with basic chores or transport.
Low Care Needs
Approx. Fortnightly Funding
$694.35
This may include additional support for individuals who need extra assistance to help maintain their independence at home.
Level 2
Level 3
Intermediate Care Needs
Approx. Fortnightly Funding
$1,511.96
This may include services and products to assist with daily tasks both at home and in the community.
High Care Needs
Approx. Fortnightly Funding
$2,292.08
This may include services for more complex needs.
Level 4
You can use the funding to pay for services such as personal care, domestic assistance, mobility support, nursing, transport, equipment, meal preparation, and continence management. Our team will help create a care plan tailored to your needs and review it regularly.
How to Arrange Funding
