Impacts to functioning (physical and/or mental) affects everyone at some point in life due to injuries or other factors at different levels.

For about 16 percent of us the impact is life long, this is commonly termed ‘having a disability’. Currently, 1 in 6 people have a disability in Australia and this figure is growing as stigmas reduce and awareness increases in our communities.

As an NDIS registered provider, our aim is to support our participants to live their life the way they want to, always have a voice, and achieve their goals!

Specialist Support Coordination

Support Coordination is a vital element of the NDIS process carried out together with and on behalf of participants. It involves you working with a Support Coordinator to identify, coordinate and access supports and services that meet your specific needs, wants and preferences. Further, where your needs are not met, to strongly advocate and support you to get the right funding you need.

Specialist Support Coordination is a higher level of support. The focus is on reducing difficulty in the participant’s support environment and helping the participant overcome immediate and/or significant barriers in their world and navigate the likely intersections with government departments, non-government departments and other networks.

At Activated Concepts, we apply our trauma-informed lens and extensive experience to partner with our participants to deliver them purposeful support coordination and proactive advocacy to ensure they have adequate plans and services to achieve their goals. Our Coordinators’ pride themselves on being available and supportive to their participants needs as things change and grow and celebrating their participant’s successes.

Although we offer a lot of NDIS Services, it is important to note that our Coordinators work independently for their participants to ensure they are offering you the best providers – that will always include external providers.

Specialist Behaviour Support and Training

Specialist Positive Behaviour Support (SPBS) focuses on evidence-based strategies and person-centred supports that address the needs of the person with disability and the underlying causes of behaviours of concern, while safeguarding the dignity and quality of life of people with disability who require support.

At Activated Concepts, we assess the interaction between our participant, the environment and other people involved with our participant in their context (not our offices). Our functional assessments, interventions, plans, data collection and training are always tailored to support behavioural change whilst empowering our participants, and their networks, to live their best life without restrictions, wherever possible.

We partner with our participants to support ongoing change, not just develop a standardised plan, and disappear. Our focus is on clinical interventions, purposeful relationships, ongoing analysis, and consistently seeking improvements to our specialist behaviour support as participants grow, change, and achieve more in their life!

Psychosocial Recovery Coaching and Assessments

A Psychosocial Recovery Coach is a specialist mental health worker that some NDIS participants with mental health conditions (or psychosocial disabilities as they are called in the NDIS) might have access to in their plans.

This is a newer support from the NDIS, and one that has been welcomed with open arms by Activated Concepts. The role of a Recovery Coach is to assist people with disabilities arising from mental health conditions to live a full and contributing life. This assistance is in all areas and is holistic.

At Activated Concepts, our Recovery Coaches are leaders in trauma-informed and person-centred practice. Their passion to walk beside participants, professional experience and advocacy skills are what sets our Recovery Coaches apart.

Daily Living and Community Access Supports

Daily living and community access funding is vital to the NDIS to support people with a disability live the life of their choosing.

Daily living supports the development of life skills that include public transport travel, banking, money and purchasing, cooking, personal hygiene, grooming, promoting personal health and wellbeing, decision making and choice. This can be provided within community settings or at the person’s home.

Likewise, community access supports the access and use of general community facilities, services, and activities to achieve and maintain real inclusion in a participant’s local and wider community. Usually, community-based activities and the supports take-up a good part of each day or as per participant’s choice and need.

We aim to support participants with their goals, and we would love to hear how we might be able to partner with you on yours!

Accommodation and Respite Services

Accommodation and Short-Term Accommodation (aka Respite) are ever changing in the NDIS. The sector is really trying to support people in new and innovative ways to have more power and choice when it comes to their home and living supports. This has led to the introduction of different funding models like Individualised Living Options (ICO) to complement the more common Supported Independent Living (SIL) options.

At Activated Concepts, Individualised Living Options and Short-Term Accommodation are areas we have been encouraged into by our participants requesting these supports from us. This was due to our consistent staffing, service and the relationships fostered from our Daily Living and Community Access Support Workers with our participants.

Like our Daily Living and Community Access Supports, our point of difference in accommodation and respite services is that you will know you workers to help you feel safe, comfortable, and respected. You will not have strangers turning up for shifts or entering your home – ever. It sounds basic but sadly it is not standard in the sector.
